A2198841 | Wednesday, 20 July, 2022 | Left flank draining tract with 3cm x 3cm area of inflammation and subcutaneous emphysema. In location of recent abscess
Radiographs- metallic object consistent with bullet in left subcutaneous flank
After OHE, the draining tract was explored by incising dorsally in skin with #15 blade and dissected subcutaneously with metzenbaums. Discovered bullet and extracted. Closed SQ with 3-0 monocryl, 3-0 monocryl skin interupted.
Assessment: Presented with abscess in left flank, no apparent external wound at that time. The ballistic impailment must have occurred several months ago and was lodged in the subcutaneous space. The skin subsequently healed over it eventually forming an abscess due to the foreign material present. Once the abscess had a surgical drain placed and the infection healed a draining tract led to the nidus of infection, the bullet. Since the bullet was lodged just under the skin no major damage ocurred. The path did not go through any vital areas.
